This program provides intake and assessment services, and Person Directed Coaching to adults aged 60 or older, living in their homes, who may be at-risk of impoverishment and out-of-home placement due to their functional challenge. Services are also provided to Veterans of any age who are referred by the Veterans Health Administration in Hawaii. The intake and assessment services offer a person-centered approach to assessment and support planning and participants are offered a choice in selecting how supports are obtained. Person Directed Coaching provides a person-centered approach to assist participants in making informed decisions regarding their care and allows them to exercise decision-making authority over a budgeted dollar amount of public funds that are used to purchase supports to meet the participant’s individual and unique needs.
