Kūpuna Outreach Support and Services is a three part program that is available to Kūpuna’s who are 55 years and older and caregivers of a Kūpuna. Services available at these sites offer Senior exercise using the Arthritis Foundation Exercise Program curriculum and usually includes an activity or health education discussion after the fitness portion is done. The Caregiver Support Group and training is offered to caregivers (CG) of persons 55 and older. The sessions run at the same time as the senior exercise so that the CG could bring their Kūpuna to exercise while they receive the support they need. Finally, the Case Management services are available on site for anyone in the groups or walk-ins. Participants receive a comprehensive assessment and create with the case manager a support plan that will help them better manage at home. This program began in July 2016 to provide outreach to the communities regularly and address gaps in services.

Eligibility:
Seniors must be 55+ for all services. Caregivers 18+ interested in the Support Group must be caring for a senior age 55+.

Expectations:
All participants should arrive to scheduled classes on time, respect staff and other participants, participate in group discussions and keep all information shared confidential. All participants should work closely and cooperate with a KOSS staff to achieve goals on their support plan.
