Community Based Crisis Group HomeThis program provides short-term, acute interventions to youth ages 12 to 20 experiencing a mental health crisis. The program is provided in a small homelike group home as an alternative to acute care or community hospital crisis stabilization. The program is designed to increase the number of women receiving prenatal care in their first trimester, improve women’s health by identifying health, economic, psycho-social and environmental risk factors, connecting the women with the necessary and appropriate resources that meets their needs, improve birth outcomes, and to enhance early and adequate prenatal, postpartum and interconception care.Services include outreach to high risk populations; initial prenatal health screen and assessments; care coordination such as referral and support to obtain and maintain early and continuous prenatal health care; case management, follow-up assessments, and support each trimester and during the six months post partum period. The program is located on the grounds of Kilauea Elementary School on Kauai and provides affordable quality preschool services to children ages 3 to 5 years old.The program targets families who meet the income guidelines on income at or below 200% of the Federal Poverty Level (FPL) but do not qualify at the more stringent income requirements of the Head Start program (at or below 100% of the FPL). The school offers full-year, full-day education and supports the increasing number of parents who are working, in training programs, attending school, or trying to enter the work force but still cannot afford quality preschool services for their children. Respite services are provided in a small homelike group home setting, and normal length of stay is two to three days. Respite services are designed to give these families short-term dependable relief to allow them to rejuvenate and provide a more stable family environment upon the youth’s return. C Kealakekua, HI 96750 (808) 323-2664 Fax: (808) 323-2999 Substance Abuse Counseling ServicesThe Substance Abuse Counseling Services program is designed for clients identified through the Department of Human Services who need additional counseling and support specific to dealing with substance abuse issues and parenting.The Substance Abuse Counseling Services program works to: • Instill a sense of hope in the family and convey the belief that changes is possible and that families can gain control over their circumstances. • Empower families to achieve their goals, direct their own lives, and become less dependent on service providers. • Teach families to advocate for themselves and to use community resources more effectively to prevent or eliminate future problems. • Help families to view challenges and crises as opportunities for growth and change. The Kauai program is part of a statewide collaboration of 11 agencies working together under a federal teen pregnancy prevention grant.
